Treatment Guarantee is required in advance of all in-patient and some out-patient treatments, as indicated by the notes 1 and 2 in your Table of Benefits. If you require any of these treatments, please download and complete the Treatment Guarantee Form from the downloads area at the bottom of this page and return it to us. Following approval, cover can be guaranteed and, where possible, we will arrange to settle your bill directly with the hospital.
Whenever your medical provider confirms to you that your treatment can’t be offered on a direct settlement basis, you will need to pay your bill directly to your medical provider and then claim back the costs from us. In order to claim for your reimbursement, please download the Claim Form from the attachment area at the bottom of this page, then complete and return it to us. Please note that reimbursements are subject to your plan’s benefit limits and terms.
